Karkass polish one is been shooting players constanting destroying their shields, blocking teleports, sabotage players progress and pushing players to their deaths he has been doing this every time

That's correct. He violates most of rules. He also uses dU prefix (without a dot but still).

saw him right now on main server and it was a mess .... again !

this guy really is a pain in the ..somewhere.
he earned his ban more than very well .. and he had his chances.
so .. i hate to say such things cuz i always believe in the 'good' lol

Nickname ban? This is not efficient enough. He wouldnt be an idiot for returning using this name (or will xD). Besides this, user ID or IPs addons would be good. If you already did it, forget what i said.
